Job Description

JOB SUMMARY:

Responsible for providing preventative, curative, and rehabilitative medical care to assigned patients and determining appropriate regimen of health care for each patient. Maintaining supervision of case management of Community Health Systems, Inc. patients including but not limited to: referral to appropriate specialists, ancillary services, arranging home care and hospital care if needed, call backs to pharmacies and patients as needed.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

1. Plan and provide the care for and the management of stabilized chronic conditions, asymptomatic and well patients of all ages and socioeconomic cultures.
2. If asked, assist in developing and implementing procedures and protocols to facilitate the delivery of comprehensive health services.
3. Assists in maintaining and implementing protocols for Quality Assurance on a routine basis.
4. Will serve, as needed, as preceptor for medical students.
5. Initiate referrals to medical specialties for assigned patients as needed.
6. Responsible for initiating referral to the emergency room/hospital or extended care facilities for assigned patients as needed.
7. If asked, assist the Quality Team in implementing and maintaining quality procedures and protocols.
8. Assist and educate other/new Providers in complying with Policies and Procedures of CHSI.
9. Maintain professional and excellent working relationship and dialogue with Site Manager/Supervisor, Registered Nurse(s), and other providers to assure smooth operation of clinic.
10. Responsible for participating in quality improvement projects/activities and accountable for quality and PCMH.
11. Every attempt should be made to solve all problems at clinic level.
12. Will report direct patient care issues to the Medical Director and any other issue(s) (i.e. administrative) to the Site Manager.
13. Will be required to attend Provider meetings and various clinic or corporate meetings, training or seminars.
14. Based on business needs, you may be required to provide coverage at other CHSI clinic locations other than your home site.
15. May be required to work weekends or evenings as designated.
16. Perform all other duties as directed either formally or informally, verbally or in writing.

SUPERVISORY RESPONSIBILITIES:

• Supervises the assigned Nurse Practitioner(s)/Physician Assistant(s) in the performance of their clinical duties and responsibilities per existing protocols.

KNOWLEDGE, SKILLS AND ABILITIES:

• Bilingual (English/Spanish) preferred.
• A high standard of professionalism and professional ethics and conduct is expected in speech, manner, attitude and appearance at all times.
• Possess strong interpersonal skills and ability to work well with others.
• Well-developed verbal and written communication skills and the ability to work cooperatively with divergent groups
• Must possess excellent customer service including verbal and written communication.
• Ability to deal effectively with changing situations and stressful environment.
• Ability to perform tasks related to physical activity to complete the responsibilities of the position.

EXPERIENCE AND EDUCATION:

• Must be a graduate from an accredited school of medicine recognized by the California Board of Medical Examiners
• Must be Board Certified or Board Eligible in Family Medicine
• Must hold a current license to practice medicine in the State of California
• Must have a current DEA certificate
• American Heart Association BLS certification required
• Must maintain continuing medical education requirements for licensure
